Health History
A student Health History is collected from the parent/guardian upon enrollment or when updating your student information in ParentVUE through online verification. If you as the parent/guardian indicate a health concern, the information is reviewed by the School Nurse. The information provided is used to assist the school Nurse in providing appropriate health services to students during the school day and to prepare staff for any emergency situations that may arise.
Life-Threatening Conditions
Students with life-threatening health conditions require medication or treatment orders in order to attend public schools. The law defines life-threatening condition as a health condition that will put the child in danger of death during the school day, if a medication or treatment orders and a nursing care plan are not in place. Examples of a life-threatening condition are: Seizure disorders, Anaphylaxis, Diabetes, SEVERE Asthma, Blood disorders, Cardiac disorders, or other life threatening conditions.
If your student has mild or moderate asthma, if you and your health care provider choose to have medication at school for your child, please use the Medication Authorization Form below. If your health care provider does not indicate what type of asthma your student has, the school nurse will contact you and your health care provider for clarification, or it will be considered life threatening until documentation is received. Contact your school nurse with any questions or concerns.
Medications
State law requires written permission from a licensed health care provider and the parent/guardian before any medication, prescription or over-the-counter medication may be taken at school. If your child needs to take medication at school, please return a completed medication authorization form.

Lice
Lice are tiny insects that live on the scalp and crawl through hair. Lice need human blood to live, and they die within 24 hours if they cannot feed. Lice lay eggs (nits) that cling to the hair close to the scalp. Nits are oval shaped and most often can be seen in the hair and behind the ears or neck. Lice hatch from nits after about 6 days and can lay more eggs after 10 days. Head lice are not a sign of uncleanliness, and they do not spread disease. The most common symptom of lice is itching the scalp.
